Please remember, your scheduled appointment time has been reserved especially for you. If you are unable to make it to your appointment, we request 2 business days' notice to reschedule. There is a $75.00 late notice fee for any changes outside of this window. We understand that unexpected events do occur, and appreciate your cooperation in this matter.
We are a Preferred Provider for the following insurance companies:
Washington Dental Service, Delta Dental, Premera, Regence, Cigna, Aetna, Assurant, Aetna Dental Access, Principal Life.
We will bill any insurance you have, as a courtesy. Here is a list of insurances we accept but are not a preferred provider with:
United Health Care, United Concordia, Humana, Met Life, Guardian, Lifewise.
Every insurance policy is different and they are ever changing. We will provide you with a general estimate of treatment needs and encourage you to contact your insurance plan to learn about any exclusions, limitations, waiting periods, and maximums. If your policy ends up not covering treatment as expected, you are financially responsible for the treatment received.
For your convenience, we accept Master Card, Visa, American Express, Care Credit, Check, and Cash as forms of payment. As a way of saying "thank you" for paying in full at the time of service, we offer a 5% cash and 2% credit card discount.
